It is recommended you print out the form and bring it with you for faster event entry times. If you plan to fill it out at the venue, the line is likely to be much longer.

We recommend you discuss other contagious airborne diseases with your vet such bordetella and canine influenza. Inform your vet that your dog will be in an area where other dogs congregate, discuss the risks associated, and if preventative treatment is right for you.
